Outcomes Assessment
Measuring Success

The Family Guide/Career Portfolio life skill and job readiness training has several points of client assessment built in.

  • Learning Objectives Signoff for each chapter in Family Guide

  • Goal-Setting Exercise Signoff for each chapter in the Family Guide

  • Learning Objectives Signoff for each exercise in Practical Exercises

  • Exercise Signoff from Practical Exercises

  • Career Portfolio Evaluation in Practical Exercises.

Common Questions:

Why use measurable learning objectives?

How do signoff sheets work?

How do I prove the client completed the module?

How do the Family Guide end of chapter goal-setting exercises work?

What goes in the client's file?

Why measurable learning objectives?

Measurable learning objectives clarify the expectation for both the trainer and the client. All activities, discussions and exercises should be able to be mapped back to a learning objective and supported by the reading materials. When a trainer is determining whether an activity is an appropriate supplement to this training, the acid test is to map it back to the measurable learning objectives.

How do signoff sheets work?

Each form is designed to itemize the measurable components of the module. Each form requires two signatures—the trainer’s and the client’s. Both parties have responsibility for the completeness of the form.

§     Learning Objective Signoffs (Family Guide & Practical Exercise workbook)

  • Your signature means you have reviewed the learning objectives with the client.
  • The client’s signature means they could execute the objectives. (In the case of low-functioning clients, this may mean you delivered the content through discussions using the discussion questions and activities. For high-functioning clients, they may be able to achieve the learning objectives though reading and minimal discussion.)

§       Goal-Setting Exercise Signoff for the Family Guide

Client’s Name: _______________________                             Today’s Date: ________

        Client’s Signature                                                    Provider’s Signature

  • Your signature means you have interviewed the client and they have explained their goals related to the itemized skills and have transferred appropriately any goals to Practical Exercise 1.
  • The client signature means it was completed in the actual Family Guide end of chapter goal- setting exercise and upon request it could be verified against the signoff sheet. (See below for detailed explanation of goal-setting exercises.)

§      Exercise Signoff from Practical Exercises

Your signature means you have evaluated the practical exercise against the criteria described on the signoff sheet and noted appropriate transfer of findings to Practical Exercise 1. If any part of the exercise signoff is incomplete—return the Practical Exercise workbook to the client and guide them to completion before signing off.

The client’s signature means they have completed the exercise and transferred appropriate items to Practical Exercise 1.

§      Career Portfolio Evaluation (PE08-09)

  • Your signature means you have viewed the career portfolio in its assembled form.
  • The client’s signature means they created this career portfolio themselves.

How do I prove the client completed the module?

The assessment documents required for completion are identified at the beginning of each module. Every document requires both a client and provider/trainer signature. The client is always to complete the exercise in their books first, then you debrief at the end of the modules. Photocopies of goal-setting exercises in the Family Guide and completed exercises in the Practical Exercises book that were completed by the client must be attached to appropriate assessment documents.

What goes in the client file?

The assessment documents are identified in each module. Copies of these assessment documents can be found on the member's-only portion of this website.

This assessment process was designed to allow the client to keep their original work and through review and summary create an outcome assessment process with clear signoffs for the client file.
